Form 5498-SA is an IRS information return used to report total contributions made to a Health Savings Account (HSA), Archer Medical Savings Account (Archer MSA), or Medicare Advantage Medical Savings Account (MA MSA) during the tax year.
The form must be filed by the trustee or custodian of the account with the Internal Revenue Service. A separate Form 5498-SA is required for each type of account maintained for an individual.

Form 5498-SA reports contribution activity for medical savings accounts and is filed by the account trustee or custodian with the Internal Revenue Service.

In the year an HSA, Archer MSA, or MA MSA account holder dies, a Form 5498-SA must generally be filed and a statement furnished for the decedent to report applicable contributions or rollover activity.

For the 2025 tax year, the deadline to e-file Form 5498-SA with the Internal Revenue Service and to furnish the participant’s copy is June 1, 2026
If the deadline falls on a weekend or federal holiday, it moves to the next business day.

What changes happen with IRS Reporting After the Death of the Account Holder?
In the year an HSA, Archer MSA, or Medicare Advantage MSA (MA MSA) account holder dies, the trustee or custodian is generally required to file Form 5498-SA and furnish a statement for the decedent.
How the account is treated depends on the designated beneficiary:
If the Designated Beneficiary Is the Surviving Spouse
The surviving spouse becomes the account holder of the HSA or Archer MSA.
An MA MSA is treated as the spouse’s Archer MSA for distribution purposes, but no new contributions may be made to the account.
If the Designated Beneficiary Is Not the Spouse (or No Beneficiary Is Named)
The account ceases to be treated as an HSA, Archer MSA, or MA MSA.
Reporting requirements still apply for the year of death.
When Must Form 5498-SA Copies Be Provided to Participants?
If you are required to file Form 5498-SA, you must provide Copy B to the participant (or decedent’s representative) by June 1, 2026.
Optional FMV Statement
You may choose to send a separate statement showing the fair market value (FMV) of the account as of December 31, 2025. While this statement is optional, if provided it should be furnished by February 2, 2026.
Special Rule When No Contributions Were Made
If both of the following conditions are met:
- You furnished the FMV statement by February 2, 2026, and
- There were no reportable contributions or rollovers for the account,
Then:
- You do not need to send an additional participant statement reporting zero contributions.
- You must still file Form 5498-SA with the Internal Revenue Service by June 1, 2026 to report the account’s FMV as of December 31, 2025.
